What is Remote Patient Monitoring?
Remote Patient Monitoring allows your Salveo Healthcare Nurse (“Nurse”) to monitor your health when they are not with you. This is done using an app and your Nurse will teach you how to use it. 

I don't know how to use apps.
No need to worry. Your Nurse will be there to assist you to access and use the app. 

Clinitouch can be downloaded on any smart device that uses either; Android 13 or above, or iOS 15 or above. The device must have an internet connection. The Clinitouch app will collect, use, and disclose your personal information input into the app in accordance with its Privacy Policy. 

When will this change happen?
The upgraded APC program will be introduced to you by your Nurse from 1st August.

I want to continue to see my Nurse monthly. Do I have to change to remote monitoring?
In the coming months, your Nurse will review and discuss any changes to the program as a result of remote monitoring and how they could be of benefit to you. If you are not wanting to participate in use of the Clinitouch app, your Nurse will continue your support without this tool.

Why is the APC program changing?
Salveo Healthcare has recently tested Remote Patient Monitoring within a group of APC program clients. This trial is showing positive results, including;
-    Program participants are contacted more often by their nursing team, 
-    Program participants are feeling more supported by their nursing team,
-    Program participants are feeling more confident to self-manage their health condition,
-    Program participants know who to contact if their health is worsening, and
-    Nurses have increased capacity to receive new referrals into the program.
