Cardiac Rehabilitation Pilot Program Eligibility Criteria
Inclusion/exclusion criteria
To be eligible, participants must:
- Be 18 years of age or over;
- Currently hold a St Lukes Hospital Product including heart and vascular (Silver & Gold cover), have served all waiting periods and are financial;
- Provide explicit written or verbal informed consent prior to enrolment;
- Be able to actively participate in and benefit from the Program;
- Not be excluded by one or more of the Program Exclusions;
Be managing cardiovascular disease with a recent acute presentation/event or cardiac procedure in the past 30-days; - Be assessed by hospital staff as safe for discharge to home and/or community setting (if referred from hospital setting).
To be ineligible for the program, a participant:
- Does not meet the Inclusion Criteria above;
- Will not benefit from participating in the Program;
- Is unable to actively participate in the Services due to barriers including, but not limited to:
- Participant does not have access to internet connection;
- Participant has technical challenges with downloading or engaging with the Cardihab digital application;
- Participant experiences one or more condition/s or social circumstances that impact their; ability to actively participate in the Program;
- Program not recommended as treatment in member’s individual circumstance;
- Hemodynamically unstable;
- Unstable angina- awaiting investigation;
- Requiring oxygen therapy at rest to maintain O2 saturations;
- Impaired cognitive status – disorientation, confusion, dementia;
- Newly diagnosed arrhythmia where medical management is still being optimised or highly symptomatic arrythmias;
- Participants on dialysis or presenting with acute deterioration of renal function.
