Terms and Conditions – “Join combined hospital and extras or hospital only cover online and go into the draw to win a double pass to a JackJumpers game Corporate Box. Plus receive up to $500 off your policy and the waiver of the 2, 3 and 6 month waiting periods on extras cover” (the “Promotion”).

1. These terms and conditions apply to the Promotion.

2. The Promotion commences at 12:00am AEDT on 28/11/2025 and ends at 11:59pm AEDT on 01/12/2025 (The “Promotion Period”).

3. The Promotor is St Lukes Medical and Hospital Benefits Association ACN 009 479 618 of 93 Cimitiere Street, Launceston, TAS 7250, Tel 1300 651 988 (“St Lukes”).

4. Participation in this Promotion shall be deemed acceptance of these terms and conditions.

5. To be eligible for the Promotion the new member(s) must:

a. join a combined hospital and extras cover or hospital only online at https://www.stlukes.com.au/, with a join effective date during the Promotion Period

b. pay the first months’ premium at the time of join;

c. be new to St Lukes, and have not held cover with St Lukes in the previous 12 months of the start date of the policy; excluding dependants and;

d. must be an Australian resident.

7. Where an eligible member transfers from another health insurer any annual limits used with the previous insurer are deducted from the new member’s available limits.

8. All extras waiting periods greater than 6 months apply.

9. When transferring from another health fund to St Lukes, continuity of cover to the equivalent level, will apply if all waits have been served with the other fund and the transfer is within 60 days of ceasing cover with the previous health fund.

10. All hospital waiting periods apply for members who are new to Private Health Insurance

11. St Lukes collects personal information on the membership application form. Eligibility for this Promotion is conditional on providing this information. The personal information collected will be used in accordance with St Lukes’ Privacy Policy.

12. This Promotion cannot be combined with any other offer, unless otherwise advised by St Lukes.

      GENERAL CONDITIONS

13. St Lukes, its contractors, employees, and agents, shall not be liable for any claims, losses, damages, injuries, costs and expenses suffered or sustained or incurred (including but not limited to indirect or consequential loss), arising out of or in any way connected with the Promotion and/or except for liability that cannot be excluded by law.

14. St Lukes reserves the right, at any time, to verify an individual’s eligibility for the Promotion (including an individual’s identity, age and place of residence) and to determine an individual(s) is not eligible for the promotion in accordance with these terms and conditions. Errors and omissions may be accepted at the St Lukes’ discretion. Failure by St Lukes to enforce any of its rights at any stage does not constitute a waiver of those rights.

15. St Lukes’ decision is final, and no correspondence will be entered into.

16. St Lukes assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorised access to, or alteration of the Promotion. St Lukes is not responsible for any problems or technical malfunction of any telephone, network or lines, computer on-line systems, servers, or providers, computer equipment, software, technical problems or traffic congestion on the Internet or any website, or any combination thereof, including any injury or damage to members or any other person’s computer related to or resulting from participation in or downloading any materials in this Promotion.

17. If this Promotion is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of St Lukes, St Lukes reserves the right, in its sole discretion, to the fullest extent permitted by law to:

a. exclude any individual; or

b. modify, suspend, terminate or cancel the Promotion, as appropriate.

19. Employees of St Lukes are not eligible for the Promotion. This includes an employee’s partner/spouse and dependant.

20. Employees of Salveo Healthcare Limited (ACN 645 287 583) (“Salveo”) are not eligible for the Promotion. This includes a Salveo employee’s partner/spouse and dependant.

21. Employees of Healthy Business (Tasmania) Pty Ltd (ABN 41 075 007 500) (“Healthy Business”) are not eligible for the Promotion. This includes a Healthy Business employee’s partner/spouse and dependant.

22. Employees of Healthcare Software Pty Ltd (Tasmania) Pty Ltd (ABN 75 094 641 755) (“Healthcare Software”) are not eligible for the Promotion. This includes a Healthcare Software employee’s partner/spouse and dependant.

 PRIZE DRAW

23. The prize for the winner of the Promotion is a 1 x double pass to attend a Corporate Box at the JackJumpers home game on Sunday 1^st February at 2:30pm at Mystate Arena in Glenorchy, Hobart. The Prize is subject to the conditions of entry to Mystate Bank Area, which St Lukes has no control over. The Prize is also subject to the Code of Conduct as specified in Corporate Hospitality – Terms and Conditions.

24 . There are three (3) winners to be drawn. The first and last names of the winners will be displayed on the St Lukes website from 05/01/2026.

25. Winning entries are excluded from the subsequent remaining draws of the Promotion. One entry is eligible for only one prize.

26. To be eligible to win the prize, the new member must be over the age of 18 and comply with points 5a-5d above. The winner’s guest must also be over 18 years of age and does not need to be a member of St Lukes. The Winners must hold an active Policy with St Lukes and not suspended or in arrears.

27. If there is a dispute as to the identity of an entrant, St Lukes reserves the right, in its sole discretion, to determine the identity of the entrant.

28. St Lukes will choose the winners of the Promotion at random from the contact details collected on the Membership Application form. The random draw will take place 05/01/2026.

29. The winners will be contacted by St Lukes directly to organise delivery and/or collection of their prize via email or phone.

30. St Lukes will not be liable for any cost incurred by the winner to attend a prize event, and for the avoidance of doubt, this includes (but is not limited to) travel and accommodation expenses.

31. St Lukes will not replace a prize that has been lost, stolen or expired;

32. The prize is not transferable, exchangeable or redeemable for cash;

33. St Lukes may conduct such further draws as are necessary if the prize remains unclaimed 2 weeks after the draw, If necessary, an unclaimed prize draw will be conducted on 19/01/2026 and that winner will be notified via email or phone.
