Basketball trading cards competition terms and conditions

Terms and Conditions – “Basketball trading cards competition to win 1 x $100 JackJumpers Store Voucher” the “Promotion”).

1. These terms and conditions apply to the Promotion.

2. The Promotion commences at 10:00am AEDT on 29/11/2025 and ends at 12:00pm AEDT on 29/11/2025 (The “Promotion Period”).

3. The Promotor is St Lukes Medical and Hospital Benefits Association ACN 009 479 618 of 93 Cimitiere Street, Launceston, TAS 7250, Tel 1300 651 988 (“St Lukes”).

4. Participation in this Promotion shall be deemed acceptance of these terms and conditions.

5. Entry into the Promotion is limited to one entry per person 

6. The Promotion draw:

a. The prize for the winner of the Promotion is 1 x $100 JackJumpers Store Voucher (the “Prize”). The use of the Prize is subject to the terms and conditions of the JackJumpers Store which the Promoter has no control over. 

b. To be eligible to win the Prize entrants must complete and submit the Promotion entry form via the QR code used solely for this Promotion;

c. St Lukes collects personal information on the Promotion entry form. Entry into this Promotion is conditional on providing this information. The personal information collected will be used in accordance with St Luke’s Privacy Policy.   

d. Incomplete, indecipherable or illegible reviews will be deemed invalid.

e. If there is a dispute as to the identity of an entrant, St Lukes reserves the right, in its sole discretion, to determine the identity of the entrant. 

f. St Lukes will draw the winner of the Promotion at random. The draw will take place on 03/12/2025 at 10:00am AEST at 93 Cimitiere Street, Launceston, TAS 7250.

g. The winner will be contacted by St Lukes directly via email and/or phone to organise delivery and/or collection of the Prize.

h. The Prize is not transferable or exchangeable.

i. Any costs incurred to claim the Prize is at the expense of the winner.

j. St Lukes may conduct such further draws as are necessary if the Prize remains unclaimed for four days after the date of the draw, or subject to any directions given by a regulatory authority. If necessary, an unclaimed prize draw will be conducted on 17/12/2025 and that winner will be notified via email and/or phone.

6. St Lukes, its contractors, employees, and agents, shall not be liable for any claims, losses, damages, injuries, costs and expenses suffered or sustained or incurred (including but not limited to indirect or consequential loss), arising out of or in any way connected with the Promotion and/or except for liability that cannot be excluded by law. St Lukes accepts no responsibility for any tax implications that may arise from the receipt of the voucher. Independent financial advice should be sought.

8. St Lukes’ decision is final, and no correspondence will be entered into.

9. St Lukes assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorised access to, or alteration of the Promotion. St Lukes is not responsible for any problems or technical malfunction of any telephone, network or lines, computer on-line systems, servers, or providers, computer equipment, software, technical problems or traffic congestion on the Internet or any website, or any combination thereof, including any injury or damage to members or any other person’s computer related to or resulting from participation in or downloading any materials in this Promotion. If this Promotion is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of St Lukes, St Lukes reserves the right, in its sole discretion, to the fullest extent permitted by law to:

a. exclude any entrant; or

b. modify, suspend, terminate or cancel the Promotion, as appropriate.

10. Employees of St Lukes are not eligible for the Promotion. This includes an employee’s partner/spouse and dependant.

11. Employees of Salveo Healthcare Limited (ACN 645 287 583) (“Salveo”) are not eligible for the Promotion. This includes a Salveo employee’s partner/spouse and dependant.

12. Employees of Healthy Business (Tasmania) Pty Ltd (ABN 41 075 007 500) (“Healthy Business”) are not eligible for the Promotion. This includes a Healthy Business employee’s partner/spouse and dependant.
