Introducing My Health Guardian – take a step to a healthier life
With modern life and busy lifestyles being what they are, many people often find they don’t dedicate enough time to looking after themselves. Indeed, many of us simply don’t have the support or resources needed to make the positive changes that will have a lasting impact on our health.
That has now changed for St.LukesHealth members with the arrival of My Health Guardian, a new preventative health program designed to help you take steps to a healthier life.
Healthways Australia delivers the My Health Guardian program to St.LukesHealth members. Healthways provides care management and wellness programs to over 38 million people worldwide.
My Health Guardian can help improve the physical, mental and lifestyle aspects of your health.

Health Guardian can really make a difference to your health and your life and best of all it's available to St.LukesHealth members at no extra cost. It's just another way we're providing value for money on your health cover.
Health Guardian program has been designed to suit the needs of all members and has two important aspects:
For healthy members
For the vast majority of members who are generally healthy, the My Health Guardian website provides a wide range of health planning and monitoring tools to help you look after your health. You can access the website through a personalised and secure member login, which will welcome you to your own individual homepage. The website is available to all members aged 18 years and over.
From here you can complete a personal wellbeing assessment, develop your own wellbeing plan, incorporating daily or weekly meal and exercise plans, with a journal to keep track of your progress, a health and wellness A-Z library as well as an online health coach on-hand to answer any questions you may have.
There are lots of other useful tools to keep you inspired and on track, including My Health Guardian tip of the day, a points system for milestones reached as well as a daily challenge for motivated members.
To get started you need:
- St.LukesHealth hospital and/or extras cover
- To be 18 years of age or over
- Your membership number and some personal details

- Choose GET STARTED to navigate to the login page
- At the bottom of the login page select 'Sign Up'
- Select SLM as your Fund
- Enter your membership number and personal details
- Select 'Create New Account'

- Take the Wellbeing Assessment and find out how healthy you really are
- Create your personalised Wellbeing Plan
- Contact a qualified health coach with any questions you have about diet, exercise or lifestyle changes
- Record your progress towards a healthier life

For members with chronic conditions
If you have a chronic condition and hold hospital cover, telephone support services from nurses trained in chronic disease management are available to you. Eligible members who choose this service will be supported by a care manager who is a registered nurse, who will work with you to design a personalised care program to suit your individual needs and lifestyle.
To join the My Health Guardian telephone support service for chronic conditions you need:
To have a qualifying chronic condition
St.LukesHealth hospital cover
To be 18 years of age or over
Care support services available include:
Telephone support from registered nurses to help improve your health condition
A registered nurse care manager will help you draw up an online health action plan, which can then be monitored for progress
Online access to approved health coaches for guidance on diet, physical activity and lifestyle
SMS or email reminders for any preventative and health screening tests you might need
Reminder calls prior to your GP appointments
Advice on local community services to help you with the maintenance of your condition The care support services are not intended to replace your GP's treatment plan. Instead they are designed to support your GP's treatment plan. Your GP will be kept informed of your progress in the program at your request.
